3. Come Prepared
Please arrive 15 minutes before your appointment time with the following:
- Photo ID
- Insurance Card
- Child's past medical history (including immunization records)
Missed Appointments:
A $75 fee will be charged for missed appointments or appointments cancelled with less than 24 hours' notice.
Late Arrivals:
We may need to reschedule your appointment and/or charge a $25 fee if you arrive more than 15 minutes late (established patients) or 10 minutes late (new patients).
